1. Introduction
The introduction provides a high level overview of what a care journey looks like, and how adult social care services work.
2. First Contact
The first contact with Adult Social Care begins with recommendations for seeking community support and guidance on where to find it, followed by an explanation of what to expect when engaging with adult social care services.
3. Care and support assessments
The “Care and support assessments” animation explain the basics of a care needs assessment, including considerations such as daily routines, personal care, staying active, and home management, while also discussing the roles of family, friends, and council-arranged advocates in providing support.
4. Your care options
This animation explains what different care options mean – including short-term care services, care and support plan, brokerage. Accommodation (Housing support options).
5. Financial assessment
This animation explains the basics of a financial assessment.
6. Paying for your care (part 1)
“Paying for your care part 1” explains various aspects of paying for care, including what a personal budget is and eligibility criteria along with how a personal budget can be used. It also covers co-funding and care costs contribution (in relation to a financial assessment outcome) and briefly references income maximisation, and access to independent financial advice.
7. Paying for your care (part 2)
“Paying for your care part 2” explains various aspects of paying for care, including information on debt responsibilities, direct payments and deferred payment. It briefly references accommodation (housing and support options).
8 Carers support
This animation defines what a carer is, and briefly discusses carer assessment, including what it involves (referencing support groups, training, balance between work and caring, practical help to support assessed needs, taking a break from caring role).
9. Safeguarding
The Safeguarding animation explains that the Council has a duty to protect vulnerable adults from abuse and neglect, and what safeguarding means. It includes a generic call to action to report safeguarding concerns or someone at risk.
10. Jargon buster
This animation has a directory of Plain English definitions of the most commonly used/misunderstood words and phrases in the adult social care journey.
